　　請假條格式：

　　標題：居中寫《請假條》，用來表明此文是用來請假的。

　　稱呼：平時怎么稱呼，這里就怎么寫，后面加冒號。這個地方往往是領導、相關負責人等。

　　正文：請假理由。直接寫請假的原因，起止時間。如果有什么事要交代，也可以在寫完請假原因之后接著寫要交代的事情。正文寫完后要加祝頌語，祝福對方，這個是所有公文里表示對對方的友好。

　　署名：寫在正文下右下方第三行處。如果是學生向老師請假，在名字前面加上“學生”二字。
